It is with deep regret that State Fire Marshal Brian S. Geraci announces the death of “Inez”, she was previously one of the three K-9 accelerant detection teams the Office of the State Fire Marshal operates. She passed away in the late evening hours yesterday, Friday, September 18, 2015 after a sudden illness. Inez passed away peacefully with her partner/handler, Senior Deputy State Fire Marshal Howard Ewing and his wife Michelle at her side. Inez should be remembered for all of her hard work and dedication to the field of arson detection and fire prevention. She has been recognized over the years for her outstanding work including her and Deputy Ewing being awarded 2007 Deputy State Fire Marshal of the Year. As a Bureau of Alcohol, Tobacco and Firearms (ATF) trained dog she also traveled far and wide on a variety of cases. It is estimated Inez worked over 500 cases in her career.

Senior Deputy State Fire Marshal Ewing and K-9 partner Inez were an exceptional tool used against arson in the State of Maryland. She was trained at the Bureau of Alcohol, Tobacco and Firearms (ATF) canine training facility in Front Royal, Virginia. Inez was a member of ATF’s Accelerant Detection Canine (ADC) Class #50. Inez successfully completed six weeks of “imprintation” training, as well as five weeks of “handler and dog” training. Canines have proven to be an invaluable tool in assisting arson investigators determining if flammable liquids were used to start a fire. Inez was trained to find ignitable liquids such as gasoline, kerosene, lighter fluid, etc. She graduated in 2005 and retired April 25, 2014.

Senior Deputy State Fire Marshal Ewing is a fifteen year veteran of the Office of the State Fire Marshal. He has been assigned to the Northeast Region, consisting of Carroll, Harford and Cecil counties for 14 of those years and was previously assigned to the Lower Shore Region.

Westminster apartment fire put out quickly; none hurt

No one was hurt in an apartment fire that was-controlled in a matter of minutes Sunday evening in Westminster, according to Kevin Dayhoff, Westminster Fire Department’s public information officer.

The fire began in the 200 block of E. Main St. in Westminster around 6:01 p.m.  According to Dayhoff, Westminster Fire Department responded to the building after a fire alarm was activated; first responders saw smoke and made entry.

Dayhoff said firefighters found moderate smoke and extinguished a fire in the first apartment at 6:29 p.m. Units from Pleasant Valley; New Windsor and Baltimore County fire departments as well as the Carroll County Sheriff’s Office and Westminster Police Department assisted.

According to Dayhoff, the fire remains under investigation by the Maryland State Fire Marshal’s Office.

According to a recent release by the Office of the State Fire Marshal:

In 2014 the Office of the State Fire Marshal celebrated its 50th Anniversary and for a little over thirty of those years the OSFM has been recognized and identified by the familiar "blue oval" OSFM patch.  

For the 50th Anniversary, Fire Marshal Geraci tasked a small cadre of sworn employees to design a special edition 50th Anniversary patch for employees and "in-house" use only.  

After designing and formally introducing the 50th Anniversary patch, Fire Marshal Geraci decided the OSFM should have a newer, fresher and re-branded  logo and the work group was again given the honorable task of designing a new OSFM patch. 

This patch was designed to be unique in its look as well as to show the OSFM is an agency under the Department of State Police.  After numerous meetings and design changes, two (2) designs were submitted to the employees of the OSFM and all employees voted for the final patch. 

     On April 1, 2015 the new logo was officially adopted for official OSFM use. You may have noticed this new logo on recently sent Notice of Investigations (NOI) and News Releases as well as our website and Facebook pages.  I wanted to forward you the new logo in JPEG format should you want to use an OSFM logo on your website or on your printed copies. We are requesting you to use this new logo and to discontinue the older "blue oval".


     Additionally, OSFM sworn police employees will soon be issued new badges that again are unique its design, clearly identifiable and can not be ordered on a "build a badge" website. Additional information may follow in a News Release in the future from PIO Bruce Bouch.

Westminster people were hospitalized with potentially life-threatening injuries Thursday evening after a kitchen fire in the 200 block of Pennsylvania Avenue in Westminster, according to a Westminster fire company spokesman.

The emergency response was quickly upgraded in response to a report that individuals were trapped in the fire.

INITIAL ON-SCENE UNIT, Initial Findings, and ACTIONS TAKEN: Westminster first arrived on the scene at 5:18 with Engine 33. Westminster assistant chief Jim Starry in command.

Upon arrival, smoke was evident from the first floor of what appeared to be a structure divided into two apartments, one on the first floor and a second apartment on the second floor.

The emergency responders from Westminster quickly identified one individual inside the structure and one individual outside that appeared to be not moving, perhaps unconscious.

Maryland State Police aviation units were requested.

Westminster firefighters rescued an unresponsive woman from the first floor and located and responded to an unresponsive male outside the dwelling.

Other than suffering from smoke inhalation, the condition of the woman was unknown. Both the male and the female were transported to Carroll Hospital.

Maryland Senior Deputy State Fire Marshall Bruce D. Bouch later reported that both individuals were treated and released from Carroll Hospital.

The fire was declared under control in a short period of time after some quick action by Westminster firefighters which quickly prevented the fire from spreading.

Units remained on the scene for about an hour. Firefighters reported that the fire may have originated in the kitchen. The source of the fire appeared to be two pots on the stove with the burners turned-on high. Firefighters turned-off the burners and ventilated the apartment.

At present, there are no reports of any emergency responders injured in the response.

In addition to Carroll County firefighters, and an initial dispatch of Maryland State Police aviation – which was subsequently terminated when it was determined that the individuals could be transported by ambulance to Carroll Hospital; officers from the Westminster city police responded as well as personnel from the Carroll County Sheriff’s Office, and the Maryland State Fire Marshalls Office.

According to Deputy Fire Marshall Bruce D. Bouch, investigators have made a preliminary determination that the fire was accidental as a result of unattended cooking on the stove in the kitchen.

Bouch further cautioned that folks ought to always stay in the kitchen while cooking. “If you have to leave the kitchen, turn the stove off,” reminded Bouch.

The incident and the fire remains under the joint investigation by the Maryland State Fire Marshal’s office and the Carroll County Sheriff’s Office.

First Female Regional Commander Takes Charge of the Upper Eastern Regional Office of the State Fire Marshal

According to the Office of the State Fire Marshal:

The Office of the Maryland State Fire Marshal is pleased to announce the appointment of Caryn L. McMahon, to the position of Deputy Chief State Fire Marshal. She has been assigned to the Upper Eastern Regional Office (UERO) located in Easton, Maryland. The office is responsible for fire investigations, inspections, plans reviews and community outreach for Caroline, Kent, Queen Anne’s and Talbot Counties.

Chief McMahon was raised in Southern Maryland and graduated from Thomas Stone High School in 2003. McMahon remains an active member of Hughesville Volunteer Fire and Rescue Squad since joining in September 2002, where she has previously held the offices of Sergeant to Assistant EMS Chief. McMahon first came to the Office of the State Fire Marshal (OSFM) as a College Intern in the summer of 2006.

While she worked through the summer, she exhibited an enthusiastic interest for the investigative field. McMahon graduated the following semester from Methodist University in North Carolina with a Bachelor of Arts Degree in Criminal Justice with a minor in Sociology as Magna Cum Laude. She was later hired by the OSFM as a Special Assistant Deputy Fire Marshal in January 2007 to perform contractual daycare inspections. She worked in that position while she pursued a sworn Deputy Fire Marshal position until she was hired in November 2007.

She graduated from Southern Maryland Criminal Justice Academy in May 2008. She has investigated and assisted with the investigations of hundreds of fires. McMahon's success has led to many arrests and convictions throughout Southern Maryland. McMahon has been nominated for Deputy Fire Marshal of the Year three times regionally, and was recognized as the State Deputy Fire Marshal of the Year in 2011. In 2011, McMahon was also selected for the Exceptional Police Performance Award by the Maryland Chiefs of Police Association.

When not devoting her time to fire investigations, prevention and awareness, McMahon is involved with Hughesville Little League Softball, where she has coached and coordinated with the league for over five years. She is also an active parishioner at St. Mary's Catholic Church of Bryantown, where she grew up in faith. McMahon and her husband, Daniel, have two wonderful children and continue to reside in Southern Maryland.

Chief McMahon brings a wealth of knowledge and experience to her new position and will be a valuable asset to the command team and the agency, stated State Fire Marshal Brian S. Geraci. DISPATCH TIME: Friday, November 21, 2014 12:47 CT: HOUSE FIRE 1299 NAUGAHYDE RD BOX: 0317 DUE: E31 E91 E23 ET44 TO3 T2 ET24 BET431 BR95 X38 CS2 - [Green, Charlie.] On box 3-17 1200blk Naugahyde Rd. E-33 on scene, working fire, (requesting) Tanker Task Force. [93]

LOCATION: 1299 NAUGAHYDE RD BOX: 0317, off Gorsuch Road near the railroad tracks and Tannery Road, Westminster, MD 21157

INCIDENT Classification:  FIRE SUPPRESSION - EMS/RESCUE

INCIDENT DESCRIPTION: At 12:46 p.m. Carroll County firefighters and EMS personnel were dispatched to a report of a house fire in the 1200 unit block of Naugahyde Road in Westminster.

INITIAL ON-SCENE UNIT and Initial Findings: Upon arrival of the first unit, WFD E-33, at 12:57, Lt. 3-1 (JE,) found the 1½ story Cape Cod house with attached garage, fully involved. Eight volunteer fire departs responded from Carroll and Baltimore Counties.

ACTIONS TAKEN: Lt. 3-1 Josh Evans assumed incident command. The building had already sustained extensive damage – collapsed roof. Initiated exterior attack. Requested tanker task force at 1:02 p.m. Fire was declared under control at 13:27 p.m.

According to a subsequent Nov. 22, 2014 report by Deputy State Fire Marshall Bruce D. Bouch, “Nearly 40 firefighters from Westminster, Reese, Hampstead, Gamber, Pleasant Valley, Manchester, Taneytown and Arcadia Volunteer Fire Departments brought the fire under control within 45 minutes after arrival.  First arriving crews found the garage and a portion of the home next to the garage, fully engulfed in flames.  No other injuries were reported during the incident. 

Bouch’s Nov. 22 report further indicated, “A team of investigators from the Office of the State Fire Marshal and the Carroll County Sheriff’s Office are actively working to determine the cause for the fire and death of the homeowner at 1299 Naugahyde Road in Westminster. 

“The 12:46 p.m. blaze on Friday, November 21, 2014, was initially called in by the homeowner, Gordon Edwin Fisher, Jr. (75), and the phone was disconnected shortly afterward.  The neighbor, a retired Baltimore County firefighter and a Carroll County Deputy Sheriff arrived first and located the victim in cardiac arrest, at the front door of the 1½ story Cape Cod style dwelling with attached garage. They quickly removed Mr. Fisher to the exterior and initiated CPR.  An EMS crew arrived from Westminster Fire Department and continued treatment; however Paramedics were unsuccessful in their attempts to revive the victim.

“The body of the victim was transported to the Office of the Chief Medical Examiner for determination of cause of death.  The origin and cause for the fire remain under investigation at this time.  Damage to the structure and contents is estimated at nearly $250,000.”

CARROLL COUNTY, MD (November 22, 2014) – A team of investigators from the Office of the State Fire Marshal and the Carroll County Sheriff’s Office are actively working to determine the cause for the fire and death of the homeowner at 1299 Naugahyde Road in Westminster.  The 12:46 p.m. blaze on Friday, November 21, 2014, was initially called in by the homeowner, Gordon Edwin Fisher, Jr. (75), and the phone was disconnected shortly afterward.  The neighbor, a retired Baltimore County firefighter and a Carroll County Deputy Sheriff arrived first and located the victim in cardiac arrest, at the front door of the 1½ story Cape Cod style dwelling with attached garage.  They quickly removed Mr. Fisher to the exterior and initiated CPR.  An EMS crew arrived from Westminster Fire Department and continued treatment; however Paramedics were unsuccessful in their attempts to revive the victim.

“Nearly 40 firefighters from Westminster, Reese, Hampstead, Gamber, Pleasant Valley, Manchester, Taneytown and Arcadia Volunteer Fire Departments brought the fire under control within 45 minutes after arrival.  First arriving crews found the garage and a portion of the home next to the garage, fully engulfed in flames.  No other injuries were reported during the incident. 

“The body of the victim was transported to the Office of the Chief Medical Examiner for determination of cause of death.  The origin and cause for the fire remain under investigation at this time.  Damage to the structure and contents is estimated at nearly $250,000.”

Carroll County Times: Fire marshals to start inspecting buildings again

It's been seven years, but the state fire marshal's office is going to start inspecting buildings again.

Allen Gosnell, deputy chief state fire marshal, said the state office approved hiring two civilians to inspect new buildings. The purpose, he said, is to free up the deputy state fire marshals to do investigations. The inspections stopped because fire marshals had too much other work to do, he said.

The deputy fire marshals are trained law enforcement officers, Gosnell said, and they have to investigate fires in Carroll, Frederick and Howard counties. Last year, they investigated 162 fires regionwide, he said. Seven years ago, they kept getting called away from inspections to investigate fires, he said, so they stopped doing them.

One of the inspectors will also work on catching up on all the businesses that haven't been inspected, Gosnell said. He said he didn't have an exact count of how many buildings there were, but said they probably numbered in the hundreds.

Mike Maring, chief of permits and inspections for the county, said the fire safety inspections will work the same way other building inspections do: He notifies the fire marshal's office, they inspect the building, and if they approve, he issues an occupancy permit.

The civilian inspectors will coordinate with the county's permits office, Gosnell said. Both of the men are experienced, he said, and are certified in fire inspections.

"They're at the top of the trade," he said.

The inspections serve two purposes, Gosnell said. First, they make sure there are escape routes for all the people using the building.

There are regulations that dictate how many exits buildings need to have, he said, as well as how large the exits need to be. The inspectors also look at how easy it is to get through the exits, Gosnell said, to make sure people will be able to use them.

The second part of the inspections looks at fire prevention, Gosnell said. The inspector will look at any potential fire hazards, as well as whether the building has properly functioning smoke detectors.

If the inspectors find a violation of code, the business has 15 days to fix the problem, he said. He said the business can also appeal the decision to him, and he'll come out and look at the problems.

"We want to work with them to make the occupants safe," he said.

Leon Fleming, Carroll County Volunteer Fireman's Association/county liaison, said he thought the new inspections would help to prevent fires. Baltimore County has a similar program, he said, and it has reduced the number of fires by pointing out hazards before they cause any damage.

He said he hoped county firefighters could go along with the inspectors, so they could get a sense of what the buildings look like, how large they are and any hazards that might be there.

That, he said, will help them fight any fire that might happen there.
